State Implementation Projects: Preventing Secondary Conditions and Promoting the Health of People with Disabilities
Description
This Cooperative Agreement provides technical assistance to sixteen State Implementation Projects, which were awarded by the National Center on Birth Defects and Developmental Disabilities (NCBDDD) for Preventing Secondary Conditions and Promoting the Health of People with Disabilities (State Disability and Health Grantees). Grantees are located at State Departments of Public Health, UCEDDs, or other university settings. (The Florida Center for Inclusive Communities at the University of South Florida was established in October 2005 as a UCEDD.)
The purpose of this project is to support States in preventing secondary conditions in persons with disabilities, improving access to public health programs, and in implementing effective health promotion and wellness programs for persons with disabilities. They accomplish their goals through the following 6 areas:
- Establish and/or sustain state offices (within the existing state health department or other appropriate location within a state government or university) for preventing secondary conditions and promoting the health of people with disabilities as a State public health priority;
- Establish and implement partnerships with universities or state health departments or other state agencies to support and complement project activities;
- Support an advisory function to coordinate and provide policy and program guidance in the State;
- Develop and/or implement a State strategic plan instrument for health promotion for persons with disabilities;
- Maintain and refine existing public health surveillance or survey activities related to disability and/or secondary conditions to target current or ongoing prevention and health promotion efforts and track and evaluate impact of program activities;
- Monitor and evaluate programs to improve the health and quality of life of people with disabilities.
